Project Overview:

 

The purpose of the study is to provide evaluation research that will guide strategic appraisal and ongoing development of the CarePartners Program (CPP) in the Christus Health System.  The project will examine economic and clinical outcomes associated with disease management counseling and support provided by Community Health Workers (CHW’s) to Christus patients with diabetes and/or cardiovascular disease (CVD).  The research will evaluate the impact of the CPP and associated activities and, at the same time, design and implement an evaluation model that can support disease management-based systemic change across the Christus Health System.
