Student Ambassador Nomination Form

Academic Year 2018-2019

 

Instructions:

The Health & Wellness Committee at Texas A&M School of Public Health was established in 2013. This committee, chaired by Dr. Marcia Ory, and represented by staff, faculty, and students from all departments was created to enhance health and wellness throughout the entire school. One of our most significant achievements was securing the CEO Cancer Gold Standard Accreditation for our workplace health promotion activities.  The committee holds monthly, work planning activities, and meetings to enhance health and wellness throughout the entire school.

Eligibility Requirements:

If you are a student and are interested in participating as a student ambassador, we invite you to complete this student nomination form. To be eligible, you must be:

  • a degree seeking student at the School of Public Health
  • in good academic standing (cumulative GPA: 3.0 or higher)
  • able to attend most monthly meetings

Applications are due by Sunday, September 30th, 2018 at 11:59pm!

If you are at the satellite campus (e.g. McAllen), then, applications are due Sunday, October 7th
